Check Your Deductions
Now that the enrollment period has ended, check your paycheck
to make sure that the proper deductions are being made.
Call MedAmerica Financial Services to correct any errors. |
Medical Savings
Account
If you enrolled in the Medical Savings Account through
Merrill Lynch and have not yet received your checkbook,
you may call Merrill Lynch at (888) 654-6837
or Chris Renner at (800) 842-2808. Checkbooks
will be sent within two weeks. |
Your Health Card
If you havent yet received your health plan card
from Performax, Care Choices, Altius or Blue Cross (depending
on the health care plan you selected), please call Denise
Clayborne at (800) 842-2808. |
College Dependents
If you have a son or daughter over age 19 and still in
college, be sure to reply to the Health Plan providers
request for college verification. If you dont, coverage
for your son or daughter can be dropped without warning! |
Vision Plan
If you have vision coverage through Vision Service Plan,
go to www.vsp.com to
find an optometrist. Make an appointment and give the
optometrist your social security number. Dependents should
also use the insureds social security number.
Its that simple! You are permitted one visit over
one twelve-month period (not the calendar year). |
Dependent Care
You are permitted to enroll in the Dependent Care Plan
or the Medical Flex Spending Account (different from a
Medical Savings Account) when you are hired or during
the annual enrollment period in December. If you missed
out during the most recent period, the only other reason
allowable is if you have a change in family status (marriage,
divorce, new baby, or spousal change in job status). |
